This is a one-time consultation service supporting Family Doctors, Pediatricians or Nurse Practitioners who will maintain ongoing responsibility for mental health care for you long term.

Fill Out Our Screening Form

Due to high demand of our services and limited personnel, we require completion of a screening form before taking on potential patients. This is to determine whether our services are a good fit for you, as some situations may require more drastic or immediate care than we are able provide. This allows us to focus our efforts on situations that are appropriate for our format.

Virtual Consultation And Assessment

We will schedule your diagnostic assessment so we can get to know you better, give you diagnostic impressions, and our treatment recommendations. Together, we’ll help you choose the right plan for your unique needs. For patients aged 18 and over, the assessments are 90 minutes. Assessments for patients under 18 last for two hours.

Limited Follow-up Sessions

We can only offer limited follow-up treatment sessions at this time— up to three 45 minute sessions over a 3-month period, due to the very high demand for our services. If we provide a comprehensive plan during this period, your primary care physician must take over this care after the follow-up treatment sessions are finished.
